The Gremlin Actua Soccer and Actua Tennis titles were both double jewelcases iirc. I don't even think they had a second disc in them, it was just for funzies...
Although reading the ones mentioned, it seems that there were some games that were released in both 'standard' and double jewelcase forms.
Of the games I've seen mentioned, I know that Ridge Racer, Tekken 2, Resident Evil 2 and Twisted Metal: World Tour all also came in standard box variants.
