Monster Hunter World (443 hours)
Overwatch (194 hours) - a little surprised Spelunky didn't beat this

Rarest trophy makes me quite sad: only 70 people have "They Walk Among Us" on 428, which is probably not even that low a completion rate among people who actually bothered playing it in the first place.

You guys clearly have much more spare time than me though, I was only about 700 hours playtime. (Is this since the system was release or only the last three years????) To be fair though, the fact I've even played modern games that long is fairly impressive in my individual case. Rarest trophy was something on JC3 that about 200 people had won.
