I look at the radar but not all attacks are obvious. Dwarfs throw bombs which don't show up when they're on the ground (and do huge damage if you get hit by a blast), bosses frequently have 'jump up into the air and land on you' moves that will get you where-ever.
Also, just fought
and basically, if the ball on chain touched you, you were dead. It does about 150 damage a hit but doesn't knock you back (possibly a bug) so it hits you 3-4 times. 1 hit kills have no place in this kind of game (they were why I stopped playing No More Heroes) .
