Circuit breakers ( ps1 ) Question

Hi when you play Circuit breakers on the playstation it says in the options to insert add on disk.

I had a copy of this years ago and i have another copy now so im sure it never came with the game. So did they ever release this add on disk in the end?

Hmm I just googled it and seen some guy showing a picture of it.

I wonder how it got released. Like maybe with a magazine or something?
Anyone Know or remember?
