AI cheating and infinte respawns. Yes i'm looking at you Call Of Duty. To me its just a lazy way for developers to not include any AI. Instead make them one hit kills and thousands of them at once. Probably been said but needs to be said again because its pathetic.

Another bad design is mainly in Fallout 3 and Oblivion. When you talk to an NPC, the background and everyone in it freezes...so if you end up clicking on someone to talk to and someone walks inbetween you and the NPC you end up looking at the back of someones head while you trying to have a conversation!!

Something that really bugs me about the dialogues in Fallout 3 is how the NPCs seem overwhelmed with catatonia the second they open their mouths. They stand there completely stock-still, arms at their sides, staring at you intently. It's very Uncanny Valley.

I found it funny in fallout3 talking to some guy about how he'd just eaten his parents, and then bedding down in the same room for the night to get some rest. Hardly a 'bug' as such but more a .. erm.. did anyone think about this or not?

I actually slept in his parent's house while their bodies and blood were strewn all about the place. It was the only non-owned bed in the area and I was desperate!

Also - sleeping on bits of cardboard on the floor in various Super-Mutant bases
