I hate Tetris, never enjoyed it so I definitely don't see it as addictive.

In fact, there's not many games that were like that for me - not since the Spectrum (Batty/Slippery Sid/Pipe Mania)

Boulderdash on the C64. And the Boulderdash Construction Kit. So many hours lost...

And then Tetris. I suppose we all had the tetris dreams... "Drop that block in there, spin that one, need a long one, there we go, yeah!" And yet, even in my dreams I never seemed to do very well... Probably says more about me than the game itself...

this one is very very difficult, but I go for SuperMario64.. I really could not stop playing this game when I had it.

This is probably the most relative question ever but here's my two personal best.

Snatcher - I've never been sure why I love this game so much. I've just never felt so immersed in the world of a game before.

VF4Evo - The only game that's ever made me get off the ass at silly times in the morning to go on long ass train rides to play other people around the country.

Can't name greatest, but most addicting to me was (and still is) UO (Ultima Online). Most expensive game I've played too (buying several versions, monthly fee)...

Baldur's Gate 2 - I bought it as soon as it came out but didn't play it for 6 months. Started playing it just when my 1st year uni exams started. Played for over 36hrs solid before one exam - then started revising and fell asleep after 5 minutes (still passed the exam i think).
Morrowind was a similar story but it happened during my 2nd year exams.

Big Red Racing was ok (funny but not good gameplay wise) - Insane is so much better.

UFO: Enemy Unknown was another game that had me completely addicted - the amount of times I restarted.
