Trauma Centre everywhere else like. I've seen it for real cheap, but if I can't play it then there's no point.

I imported it when it first came out in Japan and had no problems with it. Most missions are easily worked out and for tougher ones, trial and error will work as will any of the many FAQs available.
Only thing you'll miss out on is the story. I bought the US Wii version and whereas the missions were more or less all identical, I'd never seen the story in English and it was a nice bonus.
