It's the bosses that cause me a lot of trouble.

If you want to grind you can make as many as you want, shop bells can be found to buy upgrades and the last upgrade to max for each standard and unique weapon you have certain npc's you can buy from if you complete there quest line.

I'll take a look in to finding arms similar to Lebowski.
My attack power with the below stats just with bloodfang is 841 a hit your main right hand is only getting 341 where as your offhand is 600+, you can only hit one thing at once so dual wielding is putting burdens on your equip load and dragging down your dps. If you want an easier time id switch bloodfang to your main hand drop the second weapon and respec points from your endurance into your vigour and dex.
don't underestimate your summons too the mimic is god tier when maxed out and dose a great job of distracting bosses.
at level 122 with talismans like the scar seal and with a rune arc active Ive got
60 vigor
15 mind
36 endurance
55 Dex
55 Strength
14 int
14 faith
12 arcaine
Most bosses are going down first time with the above build and nothings really stopped me in my tracks bar that bell bearing knight in Cailed.

My suggestion would be to lower the load, when heavy your dodges are less effective and you regen stamina at a lower rate. Ditching the greave and gloves should do the trick with a negligible decrease in defence. I would also drop the Fire talisman for either the Ritual Shield or Green Turtle talisman.
Also, if you use the Bloodhound's Fang, drop Radhan's talisman for something else. And I would totally stick with that sword or other weapons that can inflict blood loss, I never quite liked weapons with split damage unless you have very high stats.

Endurance can be around 30 or 40, with talismans to increase equip load you'll be able to wear fairly heavy armour and, if you want, a greatshield.
Although I would stress that keeping mobile is a much better option.

Raya Lucaria Academy and take her a Larval Tear you can use it to reallocate your stats and respec your charcter
I've done it a few times as i wanted to tr out a few weapons and builds you should have quite a few larval tears too at this point.
